Hospice experience preferred; home health or related post-acute
experience considered Strong clinical assessment and care planning
skills Ability to manage a defined caseload independently Strong
communication skills with patients, families, and interdisciplinary
teams Willingness to participate in an on-call rotation
Responsibilities Manage and coordinate hospice care for patients
within a defined Philadelphia territory Conduct comprehensive
assessments and develop individualized care plans Collaborate with
physicians and interdisciplinary team members to ensure quality
care Educate patients and families regarding comfort-focused
treatment and support Maintain accurate and compliant clinical
documentation Participate in on-call rotation as scheduled With
