Job Description Job Description Temah Family practice is looking
for a Family Nurse Practitioner or an Adult Nurse Practitioner who
is willing to work part-time or full-time. Job Responsibilities ·
Diagnosing and treating acute illnesses, infections, and injuries
such as medical illness. · Management and treatment of common
chronic conditions such as medical illness ·
Telemedicine/telephonic consultations · Chronic Care Management of
medical illnesses · Diagnosing and Treating substance use disorder
including opioid use disorder · Medication management including
suboxone and monthly vivitrol · Ordering, performing, and
interpreting diagnostic studies. · Medication management and
reconciliation · Ordering, performing, and interpreting diagnostic
studies · Collaborating with physicians, mental health team, and
other health professionals · Referral of patients to specialist as
indicated including mental health · Referral of patients to
physical, occupation, and speech therapy (worked with Baltimore
Ortho) · Provides health education patients and families and
connecting them to necessary resources · Coordinating care to
contribute to reduction in re-hospitalization · Patient Advocacy ·
Administer age- appropriate immunization · Perform other duties as
assigned
